What does a care manager do?
Care managers work with both adults and children. You can meet with them face-to-face. They work with you to create a care plan based on your health goals. To meet those goals, your care manager can help you:
Understand your health conditions, treatments, medications and benefits
Arrange services and care with your providers
Make and keep doctor’s appointments
Find community services and support programs
Understand the KanCare system
Get regular preventive care
Learn how to better manage your health condition(s) on your own
Advocate for the care you need
To help decide which services best meet your needs, your care manager works with:

Managing chronic conditions
Some health issues need more care. If you’re managing a chronic condition, a care manager can help you get the treatment you need to feel better. They can help you manage conditions like asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart failure and more.

Are you pregnant or planning to start a family? A care manager can help you stay healthy through your pregnancy. If your pregnancy is high risk, they can help you get the special care you need. You’ll get support, including info about preterm labor and birth.
Long-term services and supports (LTSS)
Long-term services and supports (LTSS)
Are you eligible for long-term services and supports (LTSS)? If yes, you’ll get help from a service coordinator instead of a care manager. Your service coordinator will visit you at home. They’ll ask about your health and care needs. You can have family and others there when they visit.
To decide which services best meet your needs, your service coordinator will work with:

Your representative or guardian (if you have one)
Once they understand what services you need, your service coordinator takes care of setting up those services. You’ll want to see your provider for other health care needs. If you need help getting access to other health care services, your service coordinator can help. They will be there to help you for as long as you stay on the LTSS program.
