Long-term services and supports (LTSS) programs provide long-term services and supports through Medicaid managed care plans like Aetna Better Health® of Kansas. LTSS helps older adults and people with disabilities live at home for as long as they can. Health plans arrange support services based on a person’s unique needs.

What is LTSS?

LTSS is a program to help members get their health care services where they live. These are members who may need long-term health support due to their:

 

  • Age

  • Physical conditions

  • Cognitive conditions

  • Developmental conditions

  • Chronic health conditions

  • Limited ability to care for themselves

LTSS benefits

If you qualify for LTSS, you can access a range of benefits based on your personal health needs. Your needs are based upon the review with your service coordinator.

 

The services you can receive depend on the waiver you’re eligible for. They can help you live more independently, whether you live at home or in a care facility. These services include:

You’re covered for personalized services to help improve your overall health and well-being, like:

 

  • Attendant care

  • Therapy

  • Health and safety monitoring

  • Respite services 

  • Employment support

  • Housing support

You can get support for the activities you do every day, like:

 

  • Cooking and eating

  • Taking baths, grooming and getting dressed

  • Managing medication

  • Walking and getting up and down from a seated position 

  • Using the toilet

  • Driving

  • Managing money

If you qualify, you can get extra support services that may include: 

 

  • Behavioral therapy

  • Cognitive rehabilitation 

  • Physical therapy

  • Home-delivered meals 

  • Skills building for independent or transitional living

  • Occupational therapy

  • Speech language therapy

  • Supportive employment 

  • Caregiver respite (overnight and short-term)

You may be able to get help with health care tools and technology, too. Covered services include:

 

  • Medical alert rental

  • Home telehealth (install and rental)

  • Personal Emergency Response System (PERS): setup and monitoring

Your service coordinator

Your service coordinator

If you qualify for LTSS, we’ll assign you a service coordinator. This is a dedicated care manager who will visit you in person or check in with you on the phone or in other ways. Your family and friends can also be there when they visit. Your service coordinator will get to know you and your needs by working with you and your:

 

  • Friends

  • Family

  • Providers, like doctors and facilities

 

Then your service coordinator can help you find the resources you need to meet your health goals. You can call them at any time for help.

 

Care transition program

 

If you live in an institution (such as a nursing or assisted living facility), your service coordinator can help you move to a community-based living setting if needed. They can help coordinate your move and make sure it goes smoothly.

Who can get LTSS benefits?

You may qualify for LTSS if you live in Kansas and need a higher level of care. You can get LTSS benefits from the comfort of your home or assisted living center. You don’t need to live in a nursing or other facility. Talk with your service coordinator about the services you may be able to receive.

Waivers

Get extra benefits when you need them

 

Do you or a loved one need help in order to stay in your home or community? If yes, a home and community-based services (HCBS) waiver may help. You can get extra services covered if you qualify for a waiver. Those services will depend on your personal needs. 

 

There are seven types of waivers, based on these issues or diagnoses:

 
  • Autism (ages 0 to 5 years)
  • Frail Elderly (ages 65 years and up)
  • Intellectual and Developmental Disabilities (ages 5 years and up)
  • Physical Disability (ages 16 to 64 years)
  • Serious Emotional Disturbance (ages 4 to 18 years)
  • Technology-Assisted (you need ongoing care like you’d get in a hospital and rely on a medical device) (ages 0 to 21 years)
  • Brain Injury (ages 0 to 64 years)
