What’s a FIDE SNP?
A FIDE SNP is a Fully Integrated Dual Eligible Special Needs Plan (FIDE DSNP). This type of plan is for people who are eligible for Medicare and have full Medicaid coverage. Aetna Assure Premier Plus (HMO D-SNP) is a FIDE SNP plan and covers your Medicare Part A, Part B, Part D drug coverage and NJ FamilyCare (Medicaid) benefits. This includes Managed Long Term Services and Supports (MLTSS) and Behavioral Health services.
With this plan, you must use in-network providers for us to cover services. Emergencies and urgent care are covered — even when they’re out of network. You must also choose an in-network PCP for us to cover services. Referrals aren’t required to seek care from an in-network specialist.

Although Medicare is a federal program, this plan is only for people who live in our service area. This means you’ll need to continue living in one of our covered counties:
- Atlantic, Bergen, Burlington, Camden, Cape May, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Ocean, Passaic, Salem, Somerset, Sussex, Union and Warren Counties

NJ FIDE SNP members are also eligible for the CMS SEP (Special Election Period) and may switch plans as follows:
This SEP allows an individual to enroll in, or disenroll from, an MA plan once per calendar quarter during the first nine months of the year. This SEP can be used once during each of the following time periods:
- January–March
- April–June
- July–September
It may not be used in the 4th quarter of the year (October – December).
The SEP is considered “used” based on the month in which the individual makes the election (i.e., application date of the enrollment request). If the plan receives an election in March (which would be effective April 1), this counts as “using” the SEP for the 1st quarter, not the 2nd quarter. The effective date of an enrollment request made using this SEP is the first of the month following receipt of an enrollment request.
NJ FIDE SNP members may also make a change during the CMS Annual Election Period (October 15-December 7). Any changes made during the Annual Election Period will take effective January 1 of the following year.
