Members may be eligible to join Pathways to Success if:
- They are a Medicaid-eligible child (under the age of 21) living in Illinois
- A licensed provider recommended intensive home- and community-based services to correct or treat their disorder, and
- The provider completed an Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) screening
No. Members who are enrolled in Pathways to Success do not lose any benefits. You gain a set of new benefits to use along with your current ones.
Yes, you can change your organization if there is another one in your area. You can also request a new care manager within the same organization, if you wish to do so.
Yes, if you are deemed eligible for Pathways to Success but do not wish to receive more services, you can opt out at no extra cost. You can opt back in to the program at any time unless your eligibility has expired, but you must complete another IM+CANS to see if you’re still eligible.
If you’re approved for Pathways to Success, you’ll be able to use the benefits for 6 calendar months. After that, you can renew your benefits if you continue to meet the eligibility requirements.
