Ambulance rides
If you need a ride to the hospital for an emergency, call 911. We cover ambulance rides on the ground in a medical emergency for all members.
Nonemergency medical rides
We work with ModivCare to offer you rides for your nonemergency medical needs. You can get a ride for visits to your:
Provider
Dental provider
Pharmacy
Behavioral health care
There are no limits to how many rides you can use.
Get non-medical rides, too.
Your overall well-being is important to your health. That’s why we also offer members non-medical rides as an extra benefit. Your costs are covered for up to 10 round-trip rides per year for things like:
Job interviews
Job training
Getting food (from a food bank or grocery store)
Shopping for work clothes
Using community health services
Using senior services
When to ask for a ride
When to ask for a ride
Ask for a ride as early as you can, and at least three business days before you need it for regular trips
Sometimes you may ask for a ride with less notice, like:
When you need a ride home after a hospital stay
Trips for urgent health problems (not emergencies, but severe or painful enough to need treatment within 24 hours)
Need to cancel your health care visit? You’ll want to let ModivCare know before the approved and scheduled trip.

Be sure to ask for a ride at least three business days before you need it. You can create an account and sign in to the ModivCare Web Portal to schedule your rides. You can also call ModivCare at 1-877-718-4208 (TTY: 1-866-288-3133) to schedule a ride Monday through Friday, from 7:00 AM to 6:00 PM. You can also call this number for general help 24 hours a day, 7 days a week.
Be sure to have this info ready when you schedule a ride:
Your first and last name
Your date of birth
Your home address, including ZIP code, for the pickup
Your phone number
Name of your provider
Date and time of your health care appointment
If anyone is traveling with you (you can take one person as an attendant)
If you have special travel needs (example: wheelchair access)
Ride tips to remember
Rides to out-of-network providers
Are you setting up a ride outside the service area with an out-of-network provider? If yes, your doctor will need to get PA for that visit before you set up the ride. Just contact your care manager to learn more.
Get help from an attendant
Do you need someone to go with you? If you do, we’ll cover these ride costs for up to one eligible adult.
Mark your calendar
Remember to mark your calendar for the date and time of your appointment. You'll want to be ready 30 minutes before your driver is due to arrive.
Be aware of travel requirements for children
Aged 14 and younger: Must have a parent, guardian or other authorized adult with them
Aged 15 to 17: Must have a parent, guardian or other authorized adult with them or have consent from one of these on file to travel alone
No need for parental consent if the health care service is confidential in nature
