Routine eye exams are a great way to keep your eyes healthy. Enjoy covered eye exams and other vision services for children and adults. For adults, vision care is an extra benefit that goes beyond your standard health coverage.

You don’t need a referral to see a provider in our network. Providers can be family eye doctors or retail chains. Some even have offices at the mall with evening and weekend hours.
Covered services
We work with EyeMed to provide you with vision care and benefits.
For adults
Your extra benefit includes:
- $150 for eyewear (either eyeglasses or contact lenses) every 24 months
- $75 toward an annual eye exam
For kids (under 21)
Covered services include:
Routine eye exams
Two pair eyeglasses frames per year
Be sure to show your member ID card when you go to your provider visits.
