You’ll need about 30 minutes to apply. Before you start, you’ll want to have this info ready:
- Employer and income info: household monthly income, pay stubs and W-2 forms
- Social Security numbers or document numbers for each household member applying
- Date of birth for each household member applying
- Immigration info, if it applies (some noncitizens may qualify to enroll, depending on state rules)
- Photo ID if you’re applying in person
- Policy numbers for any current health insurance

Medicaid programs
You can apply:
- Online: Visit ACCESS Florida and click “Apply for Benefits.”
- In person: To get in-person help with applying, you can visit one of the Department of Children and Families’ (DCF) community partners. Just use this search tool on the ACCESS Florida website to find an office near you. Or call 1-850-300-4323. You can call Monday through Friday, 7 AM to 6 PM ET.

Florida Healthy Kids (FHK)
You can apply:
- Online: Visit Healthy Kids and click “Apply Now.”
- By phone: Call FHK at 1-888-540-5437 (TTY: 1-800-955-8771). You can call Monday through Friday, 7:30 AM to 7:30 PM ET.
- By mail: You can ask FHK to mail a paper form to your address. Just call 1-888-540-5437 (TTY: 1-800-955-8771) to ask for the form.

Do you need Long-Term Care services?
If you’re eligible for Medicaid and meet the state’s guidelines for needing a higher level of care, you may qualify for Long-Term Care (LTC) benefits. You can get them from the comfort of your home or in a nursing or other assisted living facility.
If you think you need or qualify for LTC benefits, the first step is to enroll in Medicaid. After you’ve been approved for Medicaid, the state will send you a letter letting you know if you’ve been approved for LTC benefits. From there, you can choose a managed care plan (such as Aetna Better Health®) to join.

You’ll get a welcome packet in the mail after approval of your application. The next step is choosing Aetna Better Health® of Florida as your health care plan. If you don’t choose a plan, the state will choose one for you.
