What does a service coordinator do?
Service coordinators work with both adults and children. They share info about your health conditions and ways to take care of yourself. They can also help you:
- Find a primary care physician (PCP)
- Schedule doctors’ visits
- Find specialists and behavioral health services near your home
- Get rides to your doctor, drug store or hospital
- Get medical and dental services, supplies and equipment
- Find community resources and educational materials
- Plan your pregnancy
- Manage chronic conditions
To help decide which services best meet your needs, the service coordinator works with:
- You
- Your provider
- Your representative or guardian (if applicable)
During COVID-19, your service coordinator will work with you by phone. They’ll ask you about your health and care needs. You can have family and others present, if you wish them to take part.

Service management for MSHCN and AACPA members
We offer extra care for members with special health care needs (MSHCN) and those who are part of the Adoption Assistance or Permanency Care Assistance (AAPCA) program. If eligible, you can get service management to help you plan and manage your child’s physical and behavioral health needs.
After you enroll in Aetna Better Health® of Texas, we may assign you a service coordinator. If not, then you can ask for one. Your service coordinator will:
- Find out your child’s health care needs
- Screen your child to decide if they need more help right away
- Talk with any providers, therapists or anyone else who is important for your child’s care needs
- Help you get services for your child
- Work with you and your child to make a service plan with actions and goals
- Stay in contact with you to check up on your child’s health and keep track of their service plan
